No, your isolator is completely functional and is not broken.
The rattling you hear is the sound of the high-permeability ferrite beads inside the PVC enclosure. These beads are strung over the internal coaxial conductor to suppress common-mode RF current.
To prevent physical stress, cracks, or fracturing during thermal expansion (when the unit warms up under high duty cycles or transmit power), the AA-3915 is engineered with a specific amount of mechanical clearance between the beads. They are intentionally allowed to float slightly on the line rather than being rigidly glued or potted in place.
